3. Account Information

If you are a CHOP employee, you do not need to fill out this section. You will simply use your Active Directory (AD) credentials.

Password:	CHOP employees do not need to supply a password. For all other users, passwords must be 8 characters in length and contain characters from 3 of the 4 categories: lowercase characters, uppercase characters, numbers and symbols.

Please give a brief summary of the project you are working on and the first and last names of the study managers who requested you have access to this portal.

Once this request is submitted, an account moderator will review your request and send you an email when your account has been activated.
